Résumé

La question de la prescription d’antidépresseurs dans les tableaux dépressifs de l’enfant demeure délicate par les appréhensions qu’elle suscite tant dans le chef des professionnels que dans l’entourage de l’enfant quand ce n’est pas chez le jeune sujet lui-même. S’étayant sur leur expérience clinique, les auteurs réfléchissent aux modalités qui permettent de dépasser les positions idéologiques en proposant un canevas de prise en charge se basant sur la co-intervention. En respectant le cadre des entretiens anamnestiques avec l’enfant et sa famille, trois plans d’analyse devraient nourrir la réflexion du clinicien. Ils concernent la symptomatologie regroupant les éléments du Réel de l’enfant, la souffrance et la dimension psycho-affective, le champ symbolique, ouvrant sur la sphère des significations, représentations et fonctions de la dépression. À la lumière cette investigation, on envisagerait le traitement médicamenteux lorsque le diagnostic de dépression est posé, que la souffrance du sujet est intense, que l’élaboration est rendue impossible, qu’il est fermé à toutes perspectives de rencontre d’ordre psychothérapeutique et qu’un risque de passage à l’acte existe.

Abstract

The question of the prescription of antidepressants in the depressive boards of the child remains delicate by the apprehensions which it arouses both in the head of the professionals and in the circle of acquaintances of the child when it is not at the young very subject. Supporting on their clinical experience, the authors think about the modalities which allow to overtake the ideological positions by proposing a pattern of care basing itself on the co-intervention. By respecting the frame of the conversations with the child and his family, three plans of analysis should feed the reflection of the clinician. They concern the symptomatologie including the elements of the Reality of the child, the suffering and the dimension emotional psychology, the symbolic field, opening on the sphere of the meanings, the representations and the functions of the depression. In the light this investigation, we would envisage the medicinal treatment when the diagnosis of depression is put, when the suffering of the subject is intense, when the elaboration is made impossible, when it is closed in any perspectives of meeting of psychotherapeutic order and when a risk of acting out exists.
